Surveys that Include MSU participants

Surveys that include the greater MSU community now require consideration and approval by Institutional Research.

Note: The IR Survey approval process is separate from IRB approval.


Software/IT Services and Hardware

Software and hardware purchased with personal funds CANNOT be reimbursed.

Software purchases must adhere to the MSU Information Technology (IT) Risk Assessment policies.

All software and IT services, even if they are free, must go through the IT Readiness process. The first stage of this is to determine the tier of the software or service, University Procurement and Logistics maintains a list of software/services and tiers. Tier 1 software/services do not require an IT Readiness form. Tier 2 and 3 software/services do require an IT Readiness form. Tier 1 and 2 software/services can be purchased via PCard, Tier 3 software/services must be purchased via purchase order/requisition form. We also encourage you to check the MSU Tech Store, as some Tier 1 software is available there.  

A requisition must be submitted with the completed form prior to download of free software/services. In the Notes and Attachments section of the requisition, please note that the software/service is free.


Events

Requesting Office Operations Support

If you are planning an event related to your research activities and are looking for support from Office Operations, please fill out their Event Support Request form.

Please note that the recommended lead time is a minimum of 6 weeks for events; 2 weeks for food catering requests.

If you have a recurring event, or a series of speaker events, you can use ONE form to request assistance and then attach a file with details of dates/events/etc.  Office Operations will follow up with you to personalize the support you need. 

Policies/Codes of Conduct

Proposers requesting NSF funds for a conference, symposia, or workshop to have a policy or code of conduct in place that addresses sexual harassment, other forms of harassment, and sexual assault, and that includes clear and accessible means of reporting violations of the policy or code of conduct. The policy or code of conduct must address the method for making a complaint as well as how any complaints received during the conference will be resolved. Further, the policy or code of conduct must be disseminated to the conference participants prior to attendance at the conference and made available at the conference. At MSU, the policy prohibiting sexual harassment and assault is the Relationship Violence and Sexual Misconduct and Title IX Policy. The policy prohibiting other forms of discrimination and harassment is the Anti-Discrimination Policy. A message including this code must be provided to all participants prior to and upon arrival at any NSF supported conference, symposia, or workshop hosted by MSU, including those conducted virtually. An example message is available through the Sponsored Programs Administration (SPA).

From SPA, “It is further recommended that organizers provide this information at all MSU conferences, symposia, or workshops (regardless of NSF funding).”


Cybersecurity

Cybersecurity Awareness Training is a requirement under NSPM-33 Guideline for the entire campus community. The MSU community must be prepared to protect personal and institutional data. This program also aligns with the directives of National Security Presidential Memorandum 33 (NSPM-33) and its anticipated future requirements for federally funded research and development at MSU.
